Associates Help with Fundraising Efforts!

Mission PossibleMission Possible is a way for associates to give back to their favorite Wheaton Franciscan Healthcare Foundation or community organization. While not all pledges have been tallied yet, southeastern Wisconsin associates have contributed nearly $400,000 since September 2015.

Some of the Wheaton services receiving support include: the Safe Sleep program at St. Joseph; Safe Place for Newborns; Patient Safety Fund at Midwest Spine and Orthopedic Hospital and Wisconsin Heart Hospital; and Cancer Care at Elmbrook Memorial.

One of the featured funds at St. Joseph was the Transition of Care Management (TCM) Fund, which is designed to navigate patients away from the Emergency Department (ED) to primary care physicians for non-emergency health issues. TCM meets patients’ needs, helps them overcome barriers to prevention-based health care, and provides education on the appropriate use of the ED. A team of Health Care Coordinators, Social Workers and Registered Nurses make TCM successful and improve the lives of patients.

One such patient was Lester, who came to the ED via ambulance with a high blood sugar level. He did not have a primary care physician and didn't know that he was diabetic. Through TCM he was connected with Carol, who helped him obtain health insurance, connected him with a physician, and called every few weeks to ensure his blood sugar was under control.

"She’s a wonderful person and boosted my spirits to fight this," Lester explained. "Instead of going to the ED, thanks to Carol who connected me with a doctor I like, I go to my doctor."

Carol stated, "We help patients get through everyday life so they can have a better life."

Since 2014, more than 5,000 patients have received services through TCM. This is just one of many funds supported by Wheaton associates.

In addition to making financial gifts our associates volunteer their time throughout the Milwaukee community. From assisting at walks and runs, sorting food at local pantries, adopting a highway and collecting school supplies for children in need, our associates consistently give back in many ways. Together, we make our community stronger, healthier and better.
