Wheaton Franciscan Healthcare Hospitals & Facilities Recognized for NICHE Program

Wheaton Franciscan Home Health congratulates four Wheaton hospitals and two long-term care sites for being recognized for its recommitment to the Nurses Improving Care of Healthsystem Elders (NICHE) program. NICHE was developed through the Hartford Institute for Geriatric Nursing at the New York University College of Nursing to improve the care of older adults in the continuum of care and is implemented in over 300 hospitals across the U.S.

Congratulations to the following sites that have received NICHE recognition:

At Wheaton Franciscan Healthcare, the acute care geriatric population (individuals age 65 or older) ranges from 35% to almost 67% depending on location.

“We recognize our patients in our acute care and long-term care facilities are getting older, and our nursing staff and medical leaders are committed to providing them with the best care available,” said Linda Denison Bub, MSN, RN, GCNS-BC, Director Senior Health Services, Wheaton Franciscan Healthcare. “Wheaton has been a leader in offering NICHE programing to members of its community and make up about a third of the NICHE hospitals and centers available in Southeast Wisconsin.”

As recognized NICHE hospitals and facilities, clinical leaders are trained to recognize and prevent and/or treat health problems that impact the older adult. This includes education, program development and collaboration across care sites to improve practice in the care of this vulnerable population. It also incorporates the Hospital Elder Life Program (HELP), which engages trained volunteers and expert staff to provide delirium education and prevention. The recommitment process includes completing an annual membership profile and program self-evaluation.

“We’ve seen the NICHE program make a tremendous difference in the continuum of care provided to our older patients, with trained staff to best care for their individual needs, and in turn, reduce potential readmissions,” said Bub. “The success of this program is true testament to the hard work of our medical directors and nursing leaders who have made NICHE a priority in our System.”
