Continuing Care

Wheaton Franciscan Healthcare offers outstanding transitional and extended skilled nursing care at three convenient locations in the Greater Milwaukee area. Transitional care is provided to individuals who require a few extra days or weeks of nursing and therapies, following a hospital stay or an illness. Extended care is available for those who need care for longer periods of time.

Special Programs
Wheaton Franciscan Healthcare’s transitional care bridges the gap between hospital and home for a variety of medical conditions and care needs:

  • Complex medical and post-surgical care.
  • Rehabilitative care -- speech, physical and occupational therapies.
  • Wound care.
  • End-of-life care, including hospice services provided through Wheaton Franciscan Hospice.
