Home care brings the services of health care professionals right to you, in the comfort of your own home. At Wheaton Franciscan Home Health & Hospice, we offer outstanding home health and hospice services, tailored to meet your needs and those of your family.
Home Health Services
Quality care is provided by compassionate and highly-skilled nurses, therapists, social workers and home health aides. We serve individuals living in Milwaukee, Waukesha, Ozaukee, Washington, Racine, Kenosha and eastern Walworth Counties.
Available 24 hours a day -7 days a Week
- Nursing Services: assessment, monitoring and care
- Therapies: physical, occupational and speech, adult and pediatric
- Social Work Services: help with financial planning and linking to community resources
- Personal Care: help with bathing, dressing and other daily activities
- Homemaker Services: help with laundry, meals and light housekeeping duties
- Self-Care Education: medication management and management of acute/chronic illness, such as heart failure disease and diabetes.
Wheaton Franciscan’s home care services require a physician’s orders, which we can help obtain. We accept payment from Medicare, Medicaid, private insurance, HMO/managed care and private pay.
Hospice Services
Available 24 hours a day-7 days a week
Wheaton Franciscan Hospice offers hospice and palliative care services in the home and in other settings. Care is provided by a team of experienced professionals along with trained volunteers. The goal is to achieve quality of life by promoting physical comfort and providing emotional and spiritual support.
