Evaluating Your Needs

Your intake evaluation will take about two hours. It will include detailed questions about your incontinence problems, educational materials and an evaluation using the biofeedback equipment. Treatment options will be discussed at that time.

Completed diaries if we have sent them to you
Insurance cards
List of all previous surgeries and hospitalizations
List of current medications
Physician Order (Unless previously faxed to us)
