Your Orientation
Your orientation to Wheaton Franciscan Healthcare will consist of self-study materials completed prior to your arrival and a department/facility orientation on your first day of work.
Step One: Credentialing Checklist
Before working at a Wheaton Franciscan Healthcare facility, the Non-Clinical Credentialing Checklist must be completed by the agency and faxed to the manager or supervisor of the department where contract staff or agency staff will be working, along with all supporting documents.
Step Two: Forms
The following forms must be printed, completed, signed and brought with you the first day of work in order to document your orientation.
Agency Staff Orientation Checklist
Please print a copy of the Agency Staff Orientation Checklist. Review all items listed under Orientation Materials and check them off on the list. As a non-clinical employee, you only need to complete the top portion of this checklist. (Non-clinical employees are employees who do not provide patient care.)This list must be signed, dated, and turned in to the manager or supervisor on the first day you work. At that time the site-based orientation items will be completed and signed off.
Confidentiality Statement
Please print, review, and sign the Confidentiality Statement. Bring it along on your first day of work to turn in to the manager or supervisor of the department you will be working.

Orientation Materials
Step Four: On-Site Department/Facility Orientation
Bring the completed, signed, and dated Orientation Checklist,Confidentiality Statement and Expectations for the Journey Commitment Statement on the first day you work and turn them in to the manager or supervisor of the department where you will be working. Please arrive ½ hour early to review the checklist and complete the department portion of your orientation prior to the start of your work shift unless otherwise directed.
