These individuals may be cancelled when necessary.
Your Orientation
Your orientation to Wheaton Franciscan Healthcare will consist of self-study materials completed prior to your arrival, and a department orientation on your first day of work. Expected outcomes include:

Before working at a Wheaton Franciscan Healthcare facility, the Nursing Credentialing Checklist must be completed by the agency and faxed to the central staffing office, along with all supporting documents.
If you have not completed a medication test with your agency, please print and complete this Medication Test . Please bring the completed Medication Test with you to your orientation.

Please print a copy of the LTC Agency RN/LPN Orientation Checklist. Review all items listed under Self-Study and check them off on the list. This list will be signed and dated after the department portion of orientation is completed.
Confidentiality Statement
Please print, review, and sign the Confidentiality Statement. Bring it along to the unit.
Please review the Expections for the Journey .
Please print, review and sign the Expectations for the Journey Commitment Statement. Bring it along on your first day of work to turn in to the manager or supervisor of the department you will be working.

You must complete the self study portion of the orientation prior to attending the department orientation. The department content builds on the self-study materials and you will not be able to participate effectively if you do not come prepared. Bring the completed, signed, and dated Orientation Checklist, Confidentiality Statement and Expectations for the Journey Commitment Statement with you to your first shift.
After you have completed the self-study and department portions of your orientation, you are eligible to work at a WFH facility. When you come to the unit for your first shift, you can expect to meet unit staff, tour the unit and find out some things that are specific to that unit.
When you are scheduled to work a shift, you should arrive one half hour early and complete the department orientation portion of the LTC RN/LPN Orientation Checklist. Once completed, please sign and date the checklist and the supervisor will have it placed in your file.
The following education for the Precision_Xtra Glucose_Meter (PowerPoint and Quiz) and competency (Quality Control) must be completed by all agency staff initially, after 6 months and annually. The initial education and competency are completed during the online and department orientation. Your agency will be notified of when an employee from their agency needs to complete the 6 month or the annual education and competency.
To successfully complete the Precision Xtra Glucose Meter initial orientation and competency after 6 months and annually, please review the attached PowerPoint by double clicking on the link to access the PowerPoint. Once you have viewed the PowerPoint in its entirety, you must then complete the quiz.
To access the quiz, double click on the quiz link. Please print the quiz, complete it and bring it with you to your department orientation.
Once you are scheduled to work at a Wheaton Franciscan Healthcare facility, you must be observed by a supervisor or designee properly completing a patient test and Quality Control on the glucometer. Please print the Competency Validation Tool - and bring it with you and the supervisor or designee will then validate competency and put a copy in your file.
