Wheaton MyChart & My Hospital Record

As a patient of Wheaton Franciscan Healthcare, you have two ways to securely access personal health information online:

  • Wheaton MyChart – For patients of Wheaton Franciscan Medical Group and Metro Physicians.
  • My Hospital Record – For patients who have been cared for in our hospitals or received hospital services.

Note: For patients of Wheaton Franciscan Healthcare - All Saints, hospital-based records for services received prior to January 23, 2016, are available through My Hospital Record. For services received on or after January 23, 2016, hospital-based records will be accessible through MyChart. Ask your care provider for more information.

Wheaton MyChart

For patients of Wheaton Franciscan Medical Group and Metro Physician offices, MyChart is an online tool that gives you direct and secure access to view your personal health information. This includes lab results, physician appointment information, medications, health history, immunization history, and more.

    Adult guardians may access the records of children up to age 11 or other family members who have granted legal rights to manage their health care. Their records can be linked to yours for convenient access to appointments, immunization records, growth charts, and more. Note: Access to MyChart is not available for patients aged 12-18 or their parents.

How to Enroll & Request an Activation Code for MyChart

To request access to your health information in MyChart:

  1. Complete the Wheaton MyChart Access Form at your physician's office, where you will be issued an activation code.

    : If you did not receive an activation code during your visit, please call your primary care physician to receive one or ask to sign up during your next visit.

  2. Once you receive a MyChart activation code, log in as a first-time user and create your own username and password. To help identify you, we will also ask for the last four digits of your social security number and date of birth.

My Hospital Record

For patients who received care and services at our hospitals in Southeast Wisconsin, My Hospital Record is a secure way to view your health information online.

My Hospital Record Benefits

My Hospital Record gives you direct access to the following:

  • lab results
  • medications
  • discharge and follow-up care instructions
  • health history, including immunizations
  • allergy list

How do I get access to My Hospital Record?
By providing an e-mail address to Registration when admitted, you will receive an e-mail invitation with a link to your online medical record and instructions how to log on.

What if I didn't give my e-mail address at the time I was registered?
A member of your care team will ask if you provided an e-mail address to Registration and can collect it at that time.

I already have MyChart with my Wheaton physician’s office so am I already signed up for My Hospital Record?
No, you will not be automatically signed up for My Hospital Record. At this time there are two ways to securely access personal health information online, one for medical group information and another for hospital stays.

Will my e-mail address be sold to others?
E-mail addresses are confidential and are not sold to any other entity.

What is the difference between MyChart and My Hospital Record?
MyChart is the patient portal for our medical groups, Wheaton Franciscan Medical Group. My Hospital Record is the patient portal for your hospital stay(s).

Why do I have two different online charts?
When we implement Epic Enterprise, we will be able to offer one consolidated record. At this time, we are working with two different vendors.

How does My Hospital Record work for pediatric patient or emancipated minors?
For minor children under the age of 12, records will be available in My Hospital Record. If you are a parent/guardian who wants to have access to these records, you will need to either call Relay Health customer support at 1-866-735-2963 or e-mail

You will then need to answer questions that prove you are the patient’s parent/guardian, and then Relay Health will assist you in setting up an account if the baby was not born here or if you have never been a patient. Your account would then be linked to the child’s records, and you will have access until they reach 12 years of age.

Note: Patients/Guardians of children ages 13-17 will not have access to the child's My Hospital Record per regulation standards at this time.

Within Relay Health is there any other language capability?
English is the only language in Relay Health.

What if I have issues with the Relay Health website?

I have never been an inpatient, but I had an outpatient procedure like an endoscopy. Would I have My Hospital Record?
No, only inpatient hospital visit information will be available in Relay Health.

If I want to view my outpatient records, where should I go?
You will need to contact the Health Information Management (HIM) Department.
