Wheaton Franciscan Healthcare offers outstanding patient-centered transitional and long term care at three convenient locations in Southeast Wisconsin. Transitional care is available for those individuals who require a few extra days or weeks of nursing and therapies following a hospital stay or an illness. Extended care is available for those who need care for longer periods of time.

Geriatric nurse specialists and the Hospital Elder Life Program can assist geriatric patients with dementia. Each facility provides in-house therapy and activity programs that are customized to meet resident needs and also offer comprehensive patient and family education.
