The Family Care Center–a family medicine practice–has provided high quality health care to the North Shore and northern Milwaukee for more than 35 years. This joint collaboration between the Medical College of Wisconsin and Wheaton Franciscan – St. Joseph Campus offers family-centered care by family medicine residents and attending faculty physicians.

Now in Our New and Improved Facility! Now Accepting New Patients!

Our brand new facility opened on the same property in July 2012. The building includes 25,000-square feet of space includes:

• Single-story physician clinic with 32 exam rooms
• 3 procedure rooms
• Education center for health, wellness, and physical movement classes
• Teaching / demonstration kitchen
• Seeds of Hope education garden for providing lessons in how to grow healthy food and nutrition education

A patient-centered medical home is a model of care that strengthens physician-patient relationships to encourage long-term relationships and coordinated care.
