Wheaton Franciscan Healthcare is pleased to provide wellness services to Southeast Wisconsin employers.
Our Mission “to provide exceptional and compassionate healthcare service for the well being of the people we serve” lends to enhancing benefits that are already in place or for implementing a new health and wellness initiative. We believe that our collaboration with many entities offers the best of all services wrapped into a comprehensive and custom designed package that will enable our business partners to improve the overall health status of their insured work groups as well as family members, and also save dollars in health care costs.
Our program focuses on: continuity of care, multiple options for a health risk assessment tool, additional resources for wellness education, and most importantly, excellent preventative patient services through on site coaching models. Our experience has shown the ability for the on site registered nurse health coach to save significant dollars in health care costs. Wheaton Franciscan Healthcare, through our Healthy Connections™ program, is committed to providing employers with high quality patient care in a proactive and cost effective model with added convenience for their associates and families.
