If hospitalization is required
Elmbrook Memorial Campus offers a continuum of care to seniors, including individual therapy, case management and inpatient care. Much of our inpatient care is provided by Hospitalists, physicians who specialized in the care of hospitalized patients. Our Hospitalists, nursing and support staff are very familiar and experienced with the physical, emotional and psychological changes that may occur with older patients in an inpatient setting. Our medical and surgical units, as well as our Inpatient Rehab unit, are all designed with senior health in mind. Patient and family communication is a top priority. Social work services (case management) are provided to ensure that care is continued after discharge.

NICHE Program – The Elmbrook Memorial Campus participates in the national Nurses Improving Care for Health System Elders (NICHE) program, which offers care models for providing exemplary health care to seniors.
HELP Program – The Hospital Elder Life Program (HELP) is a research-based program designed to improve the hospital experience for older adults, and prevent and recognize delirium. The program consists of a team of geriatric professionals and trained volunteers that work closely with the hospital staff to meet the special needs of the older adult patient. Learn more about HELP.
