Please complete each "Step" as you prepare for your clinical experience within Wheaton Franciscan Healthcare.
Request Forms:
- Clinical Student Request form (Nursing, Preceptorships, Respiratory Therapy, Medical Assistants, Dental Hygiene)

Step 1
Access and read the Orientation Manual.
Step 2
Access and print a Student-Instructor orientation checklist for you and each of your students.
Step 3
Access and read the appropriate OSHA and safety information for the area you are assigned to Iowa Safety
Step 4
Access and read the Roles & Responsibilities
Step 5
Review the content for the following items and forward a signed form when indicated to the Academic Program Coordinator within 1 week of the clinical start date.
Each instructor and student must have a signed confidentiality and anti harassment statement on file with the Academic Program Coordinator.

Please print and sign the Expectations for the Journey Commitment Statement.

Access and read the Unapproved Abbreviations.
Step 8
Review Electronic Health Record Education
Step 9
INTERNS ONLY - Access and print the Health Requirements which are required prior to your education experience.
